The following materials must be read, understood and acknowledged prior to any athletic tryouts at De Smet Jesuit High School:

2. MSHSAA How to Maintain and Protect your High School Eligibility
3. De Smet Student-Athlete Handbook and Code of Conduct
The following forms must be completed and on file at De Smet Jesuit High School prior to any athletic tryouts or P.E. participation:
1. Pre-Participation Physical Examination Form, Student and Parent Eligibility Authorization Statement
2. Athletic and PE Parent Permission Form
